All packages will be paid in full at the time of sign up, all additional appointments will be pre-booked at your first scheduled appointment. Packages are non-refundable and non-transferable.

Monthly membership payments or if paid in full are non-refundable. Any member, who is under the age of 18, must have a parent or legal guardian co-sign and be present during all office visits.
